Whose address do you put on top on a formal letter?

I have to write a letter to school director requesting something, and I was wondering whose address goes on the very top of the page? it's usually centered on top.

is that supposed to be my address or the recipient ?

ex:

you deleted the quesiton and i took like 5 min. revising it SO i felt an obligation to post the answer SOMEWHERE or i would feel like i just wasted 5 minutes >_< so yeah call it ocd hahahha

Original : I currently have a 3.5 GPA. I will be applying to Depaul University this month. With this F taken out, my GPA should be higher further increasing my chances of getting accepted to Depaul.

bigger words make you sound more intellighen and make your letter more persuading and convincing .

Revised: Sometime this month i will be applying to Depaul University, *CITY/STATE*. At this point my gpa is a 3.5 and if this F is eliminated from my grades, my Gpa will increase and furthermore greatly improve my chances of being accepted to Depaul, which i would appreciate to the utmost.
